Medical Directorship

Discharge Planning
Physiatrists coordinate discharge planning for smooth transition to care - home-based rehab, outpatient therapy, or long-term facilities. They collaborate with the team for services, equipment, and follow-up to aid recovery.
Facility Presence
Rounding regularly up to 7 days per week on each patient as necessary to evaluate medical and rehabilitation progress.
Communication
Frequent and early family, caregiver & social worker communication and coordination with the interdisciplinary rehab team for the smoothest transition of care with a focus on safe patient discharges and best outcomes.
Medical Management
Physiatrists manage patient medical care, including medications, tests, and coordination with specialists for optimal health and well-being during rehabilitation.
Improve Quality Metrics
Implementing evidence-based guidelines and best practices in patient care to optimize key performance indicators including patient satisfaction scores, functional outcomes, reducing discharge to acute, maximizing discharge to community, and more
Patient Assessment and Evaluation
Assess patients to determine their rehabilitation needs, functional abilities, and goals. They conduct comprehensive evaluations to understand the patient's medical history, impairments, limitations, and potential for recovery.

Clinical expertise and educational support to the facility’s care team including administration, physicians, nursing, rehab team and staff.
